Introduction: Cardiovascular diseases are the leading cause of morbidity and mortality worldwide, with a particularly high impact in low- and middle-income countries. Cardiovascular surgery plays a key role in the management of multiple complex conditions; however, Colombia lacks a standardized national registry to comprehensively characterize cardiovascular surgical practice. Objective: To present the rationale and methodological design of the National Registry of Cardiovascular Surgery (RUNCAV), which aims to generate epidemiological and clinical information to support clinical decision-making, public policy development, and quality improvement in cardiovascular care in Colombia. Methods: This is an observational, prospective, multicenter study including adult patients undergoing elective, urgent, or emergency cardiovascular surgical procedures. Data collection is conducted between 2024 and 2026. Sociodemographic, clinical, surgical, and in-hospital outcome data are collected using the REDCap platform, administered by the Research Department of the Colombian Society of Cardiology and Cardiovascular Surgery. Data analysis includes descriptive statistics and multivariable models. Results: RUNCAV will represent the first prospective, multicenter national registry of cardiovascular surgery in Colombia, generating reliable, internationally comparable data that are representative of the national context. Conclusions: RUNCAV is the first structured national initiative in Colombia aimed at generating standardized, high-quality data. This registry will enable the development of locally adapted risk prediction models, the design of targeted interventions to reduce health inequities, and the support of evidence- based public policymaking, with potential impact on patient safety and clinical outcomes.
