Introduction: Advanced heart failure often requires advanced therapies, including heart transplantation (HTx). In Colombia, national-level data on HTx is scarce, limiting the ability to optimize patient care and evaluate outcomes. Objective: To establish the Colombian Heart Transplant Registry (RECOLTRACA), designed to consolidate demographic, clinical, and outcome data from adult and pediatric recipients in order to strengthen scientific knowledge and inform decision-making in this population. Methods: RECOLTRACA is an observational, analytical, ambispective (retrospective and prospective), multicenter cohort study that will include adult and pediatric patients who undergo HTx in Colombia and attend post-transplant follow-up programs nationwide. Follow-up will be conducted at 1 month, 6 months, and annually after HTx. Donor and recipient data will be extracted from medical records and entered into the Research Electronic Data Capture (REDCap) electronic database. Results: This registry represents the first national initiative specifically focused on characterizing adult and pediatric Colombian patients undergoing HTx. Conclusions: The RECOLTRACA registry will provide comprehensive national data on heart donation and transplantation, fostering scientific knowledge and supporting the development of strategies to improve care, clinical decision-making, and short- and long-term outcomes in Colombia and across Latin America.
